The diagnosis and management of sinus headache
The so-called ‘sinus headache’ is commonly associated with pain related to acute or chronic rhinosinusitis. According to the International Classification of Headache Disorders, 3rd edition (ICHD-3), in effect since 2018, the recommended terminology is ‘headache attributed to disorders of the nose or paranasal sinuses’. The diagnosis requires confirmation of a causal and temporal relationship between headaches and the underlying sinonasal disorder.
In clinical practice, headaches reported by patients as ‘sinus-related’ are often attributable to neurological conditions, such as migraine or tension-type headache. A proper differential diagnosis based on the current criteria and clinical guidelines is essential for selecting an appropriate and effective treatment option.
