Uszkodzenie części błoniastej przegrody międzykomorowej z wytworzeniem istotnego przecieku między lewą i prawą komorą jest rzadkim, ale poważnym powikłaniem zapalenia wsierdzia lub sporadycznie chirurgicznej implantacji zastawki aorty. W zależności od wielkości przecieku chorzy mogą nie mieć objawów lub prezentować różnie nasilone cechy niewydolności serca. Reoperacja jest obarczona dużym ryzykiem, dlatego podejmowane są próby przezskórnego zamykania tego typu ubytków. Opisano skuteczne użycie zarówno zatyczek dedykowanych do wrodzonych ubytków przegrody międzykomorowej [33-35], jak i korków embolizacyjnych [36].

Tętniaki rzekome kurtyny mitro-aortalnej

Kurtyna mitro-aortalna (mitral-aortic intervalvular fibrosa, MAIVF) jest włóknistą, praktycznie nieunaczynioną strukturą między przednim płatkiem zastawki dwudzielnej a płatkiem niewieńcowym zastawki aortalnej. Rzadko występujący defekt strukturalny tej okolicy – tętniak rzekomy (pseudoaneurysm of MAIVF, P-MAIVF) – został po raz pierwszy opisany przez Waldhausena w 1966 r. [37]. Struktura ta może rozprzestrzeniać się wzdłuż lewego przedsionka, tętnicy płucnej i aorty w górę i jest ograniczona blaszką trzewną osierdzia. Czynnikami predysponującymi do powstania P-MAIVF są: zapalenie wsierdzia i osłabienie tkanek interwencją kardiochirurgiczną w okolicy kurtyny mitro-aortalnej (zwykle implantacja protezy zastawki aortalnej, ryc. 5A, B). Z reguły to właśnie objawy związane z aktywnym bakteryjnym zapaleniem wsierdzia dominują w obrazie klinicznym, jednak u chorych, u których nie stwierdza się czynnego zakażenia, na pierwszy plan wysunąć się mogą dolegliwości powodowane niewydolnością serca lub uciskiem sąsiadujących struktur (np. naczyń wieńcowych) przez P-MAIVF [38]. Możliwe konsekwencje tego powikłania obejmują perforację tętniaka do lewego przedsionka lub aorty z wytworzeniem przetok, pęknięcie do jamy osierdzia oraz zatory obwodowe (szczególnie do ośrodkowego układu nerwowego) materiałem z jamy tętniaka (skrzepliną). Leczeniem z wyboru jest operacja kardiochirurgiczna, nawet przy bezobjawowym przebiegu (co wynika z dużego prawdopodobieństwa pęknięcia tętniaka) [17]. Opisano również przypadek skutecznego przezskórnego zamknięcia szyi P-MAIVF zatyczką do VSD [39].
