Podsumowanie

Bezdyskusyjnie najważniejszą metodą zapobiegania SCD o udokumentowanej skuteczności w prewencji pierwotnej i wtórnej (20% redukcja śmiertelności całkowitej) jest implantacja ICD, przy czym na zmniejszenie ryzyka SCD wpływa także optymalne leczenie choroby podstawowej, w tym rewaskularyzacja wieńcowa [16]. Należy ją przeprowadzać zawsze, gdy prawdopodobieństwo poprawy funkcji mięśnia sercowego jest duże, pamiętając, że w przypadku bezwzględnych wskazań do implantacji ICD decyzja o działaniach naprawczych na tętnicach wieńcowych nie powinna zmieniać pierwotnego postępowania, czyli wszczepienia ICD, ponieważ odłożenie lub dyskwalifikacja z zabiegu implantacji zwiększa ryzyko nagłego zgonu sercowego.
