ABSTRACT
Hyperviscosity syndrome as a life-threatening emergency
Hyperviscosity syndrome (HVS) is a life-threatening condition with increased blood viscosity as the underlying cause. It occurs in patients with a history of hematologic malignancy associated with excessive secretion of paraproteins (i.e., IgM, IgG, and IgA), cryoglobulins and hemagglutinins that form aggregates of red blood cells and impair microcirculation and normal tissue perfusion. Blood hyperviscosity syndrome most often occurs as a result of hypergammaglobulinemia associated with Waldenstrom’s macroglobulinemia (WM) (10-30% of patients) and multiple myeloma (PCM, plasma cell myeloma) (2-10% of patients), but cases have also been described among patients with rheumatoid arthritis, polycythemia vera, plasma cell dyscrasias, congenital spherocytosis and other bone marrow malignancies.
