Non-surgical treatment of retrognathia with the lingual appliance and the Herbst appliance

A 30-year-old female reported to an orthodontic practice with an orthodontic problem and missing teeth. Work-up revealed major retrognathia, a deep bite, a brachycephalic profile and agenesis of the lower second premolars. As the patient had diabetes mellitus and only a small amount of bone was available, she did not consent to having implants in place of the missing teeth. A consulting implant specialist also recommended finding another solution for her problem. Moreover, she did not agree to undergo an orthognatic surgical procedure as treatment. She was offered orthodontic treatment for her upper and lower teeth. It comprised the correction of the orthodontic defect of retrognathia and deep bite and closing the gaps of the missing teeth 35 and 45 with a customized WIN lingual appliance together with Herbst’s telescopes for positioning the mandible forwards.
