The diagnosis of acute coronary syndrome: a new or prior myocardial infarction?

In the daily clinical practice, there is a common problem about distinguishing between acute myocardial ischemia and prior myocardial infarction. An ECG test is the basic diagnostic tool for assessment of patients with coronary artery disease. The interpretation of ECG results can be difficult, especially when acute coronary syndrome is suspected. In this context, of essential importance in making the right therapeutic decision are ECG-based criteria for diagnosis of acute coronary syndromes, the electrocardiographic evolution of myocardial infarction and features indicative of a prior myocardial infarction. An ECG-based evaluation should be always performed in the context of the patient’s clinical condition, especially when it is necessary to establish a differential diagnosis between acute coronary syndrome and prior myocardial infarction. Therefore, it is highly important to remain vigilant about symptoms indicative of acute ischemia. In many cases additional results of biochemical and imaging tests are required to make a correct diagnosis.
