1. Marskość wątroby stanowi końcowe stadium wielu przewlekłych chorób wątroby. U dzieci dominują schorzenia cholestatyczne i choroby metaboliczne. O rozpoznaniu marskości decyduje badanie histologiczne bioptatu.

2. Początkowe objawy chorób prowadzących do marskości wątroby często są nieswoiste (uczucie zmęczenia, osłabienie, utrata łaknienia, nudności, bóle brzucha, świąd skóry). Dokładne badanie przedmiotowe może ujawnić takie odchylenia, jak żółtaczka, powiększenie obwodu brzucha (wodobrzusze, hepatosplenomegalia), poszerzenie siatki naczyń na skórze brzucha, obrzęki obwodowe, zmiany skórne (przeczosy, pajączki naczyniowe, rumień dłoni, kępki żółte, wybroczyny, nadmierna pigmentacja), ginekomastia czy hipogonadyzm.

3. Badania laboratoryjne są pomocne w ocenie czynności syntetycznej (układ krzepnięcia, stężenie albumin) i wydzielniczej wątroby (stężenie bilirubiny, aktywność GGTP i fosfatazy zasadowej). Podwyższenie aktywności aminotransferaz świadczy o uszkodzeniu hepatocytów.

4. W badaniach obrazowych stosujemy USG z oceną przepływów krwi metodą Dopplera, sonoelastografię, tomagrafię komputerową, magnetyczny rezonans jądrowy.

5. Takie powikłania, jak wodobrzusze, krwotok z żylaków przełyku, encefalopatia, zespół wątrobowo-nerkowy czy wątrobowo-płucny mogą być pierwszym objawem uszkodzenia wątroby.

6. Przeszczepienie wątroby stanowi jedyną skuteczną metodę leczniczą marskości wątroby.
